Job summary

Optum Home & Community Care is hiring for House Calls Advanced Practice Clinicians (NPs or PAs) to perform in-home health assessments across Medicare Advantage and other plan members. This field-based role emphasizes education, gap closure, and collaboration with primary care providers.

You will work in patients' homes, documenting findings, addressing social determinants of health, and helping patients access care.

Qualifications

• Active, unrestricted state license to practice in the state of employment.
• NPs must hold ANCC or AANP certification; PAs must hold NCCPA certification.
• Ability to navigate varied home environments and conduct assessments.

Responsibilities

• Conduct comprehensive in-home health assessments for Medicare Advantage and other plan members.
• Identify and document diagnoses for care management and treatment planning.
• Educate members on disease processes, medications, and compliance.
• Coordinate with primary care providers to close gaps in care.
