Job summary

Matrix Medical Network is seeking a Nurse Practitioner to perform comprehensive in-home health assessments, reviewing health histories, medications, and risk factors at the member’s residence.

Qualifications

• Master’s Degree or commensurate experience and NP licensure.
• Current NP licensure in state of practice including prescriptive authority.
• Board certification in Adult, Adult Gerontology, Family, Emergency, or Acute Care.
• Current BLS/ACLS or CPR certification.
• 1 year NP experience preferred; new grads encouraged to apply.
• Valid state driver’s license for residence.
• Auto insurance coverage or ability to obtain.

Responsibilities

• Conduct adult/geriatric NP health assessments at members’ homes to review medical history, diagnoses and treatments, and health education.
• Coordinate with physicians, case managers, social workers, and family members; document findings per Matrix guidelines.
• Administer and order point-of-care and lab testing as needed.
• Collaborate with PCP on education and follow-up; provide necessary referrals.
• Deliver services at members’ residences, including Home or Skilled Nursing Facility settings.
