Job summary

Matrix Medical Network seeks a Nurse Practitioner to perform comprehensive in-home health assessments for members, reviewing health history, medications, and risks. Visits last up to one hour, conducted at the member’s home or SNFs. You’ll collaborate with PCPs, case managers, and care teams, provide education, and help close care gaps. Flexible scheduling, weekend coverage, and FT opportunities are available.

Qualifications

• Master’s degree in nursing and NP licensure in state of practice.
• Board certification in an approved NP specialty (AANP/AACN/ANCC).
• Current BLS/ACLS or CPR certification.

Responsibilities

• Perform adult/geriatric assessments including medical history, diagnoses, treatment plans, and follow-up.
• Administer and order point-of-care and lab-based tests as needed.
• Collaborate with physicians, case managers, social workers, and other care team members.
• Provide in-home services at the member’s residence or Skilled Nursing Facility.
• Maintain clear documentation per Matrix guidelines.
