Job summary

EMPACT-SPC, part of La Frontera, seeks a High Needs Case Manager in San Tan Valley, AZ. You will provide direct crisis intervention, transportation, and family education to high-need children and families, managing a caseload of 15 families with a focus on intensive service delivery.

Requirements include BHT or higher in behavioral health, 1 year+ experience, valid AZ driver license, fingerprint clearance, and flexible hours including evenings. Bilingual Spanish is a plus.

Qualifications

• Must meet Behavioral Health Technician (BHT) requirements.
• Behavioral health experience is required.
• Bi-lingual (Spanish) a plus.
• FingerPrint clearance card or eligibility to obtain one.
• Valid AZ driver’s license with no major infractions in 3 years.
• Regular access to reliable transportation.
• At least 21 years old at time of employment.

Responsibilities

• Provide direct in-home, in-office, in-school, and community services including crisis intervention and transportation.
• Facilitate and document family, youth, and adult team meetings; engage and invite natural supports; make referrals.
• Deliver approx. 40 hours per week with 30 hours of direct client contact; maintain clinical documentation.
• Coordinate with Clinical Coordinator/Director and other departments for continuity of care.
• Develop relationships with state and community partners; ensure timely service provision.
• Work flexibly including evenings or weekends as needed; attend trainings.
