Qualifications

• Active Ohio RN license required.
• At least 1 year of acute care or medical-surgical nursing experience.
• Hospice experience preferred.
• Proficiency with Windows, Microsoft Office, and EMR systems.

Responsibilities

• Provides ongoing assessment and nursing care to patients in homes and health care facilities.
• Develops and implements a comprehensive plan of care addressing patient needs.
• Ensures adherence to current hospice standards and regulations.
• Communicates with other health care providers about changes in condition and patient needs.
• Oversees the implementation of the patient’s Plan of Care and prepares for IDG meetings.
