Otterbein SeniorLife is seeking a Hospice RN Case Manager to oversee a primary caseload and deliver skilled nursing care within the Hospice Medicare framework. The role supports patients, families, and caregivers, aligning with our mission-driven, person-centered care philosophy.

Full-time position in Ohio with on-call and weekend rotation. RN license in Ohio and hospice/kindred experience are preferred to join our compassionate team of Partners.

Qualifications

• Bachelor's degree in nursing required or preferred.
• Graduate of approved nursing program.
• Valid Ohio RN license required.

Responsibilities

• Manage a primary hospice caseload and provide nursing care per physician orders and hospice guidelines.
• Collaborate with patients and families to plan care and support during on-call coverage.
