Ohio Living is seeking a Registered Nurse Case Manager for Hospice to plan, coordinate, and deliver high-quality hospice care in patients’ homes and affiliated facilities. The role emphasizes thorough care management, timely communication with care teams, and strict adherence to regulatory standards.

The position requires Ohio RN licensure, 1 year in acute care/medical-surgical nursing, and proficiency with EMR and Microsoft Office; hospice experience is preferred.

Qualifications

• Current unencumbered Ohio RN license required.
• 1 year experience in acute care or medical-surgical nursing required.
• Hospice experience preferred.
• Proficiency with Windows, Word, Excel, PowerPoint and internet required.
• EMR experience preferred.
• Must read/write/speak English.
• Valid driver's license and insurable as per policy.
• Work may involve weekends/evenings/holidays as scheduled.

Responsibilities

• Provide ongoing nursing care and assessment in patients' homes and care facilities; develop and implement care plans.
• Follow nursing standards and hospice protocols; ensure symptom management and patient comfort.
• Communicate changes in patient condition with other health care providers; educate patients and families.
• Oversee plan of care implementation and IDG preparation; reconcile medications and anticipate needs.
• Support quality/compliance through accurate record-keeping and adherence to regulations.
