Responsibilities include assessing patient needs, developing care plans, managing symptoms, coordinating with the care team, and ensuring compliance with standards. Must hold Ohio RN license, at least one year in acute care, and proficient in Windows/Microsoft Office and EMR.

Qualifications

• Current unencumbered RN license in Ohio.
• At least 1 year of acute care experience.
• Proficiency with Windows and Microsoft Office; EMR experience preferred.
• Valid driver's license and insurable driving record.

Responsibilities

• Care Management and Service Delivery—planning, assessment, and delivery of hospice care.
• Quality and Compliance Management—maintain accurate records and regulatory adherence.
• Collaborate with health care providers; educate patients and families as needed.
• Perform other duties as assigned.
