Job summary

Qualifications

• Must hold an unencumbered Ohio RN license.
• English language fluency required.
• Proficiency with Windows, Office suite and internet.

Responsibilities

• Provide ongoing nursing care and assessments in patients' homes or facilities, and develop care plans.
• Deliver symptom management and comfort measures per hospice standards.
• Communicate changes in condition to other health care providers.
• Assess patient and caregiver needs; provide education as needed.
• Oversee Plan of Care implementation and evaluate effectiveness.
• Prepare for IDG by reviewing Plan of Care and reconciling medications.
• Maintain patient records and support quality improvement initiatives.
• Perform other duties as assigned.
