Job summary

Caretenders Hospice, part of LHC Group, is seeking an RN Case Manager to deliver and evaluate the Plan of Care for hospice patients in Ohio. This PRN/inconsistent role involves in-home visits and coordinating with the interdisciplinary team to support families facing end-of-life decisions.

Requirements include an active Ohio RN license, 1+ year clinical hospice experience, and a valid driver’s license with reliable transportation. CPR certification is preferred or obtainable within 90 days.

Qualifications

• Current and unrestricted RN licensure in Ohio.
• At least 1 year of clinical hospice/nursing experience.
• Valid driver's license and reliable transportation or public transit access.
• CPR certification preferred or obtainable within 90 days.

Responsibilities

• Makes initial nursing evaluation within 48 hours of referral.
• Identifies physical, psychosocial and environmental needs; reassesses every 15 days.
• Initiates and coordinates the plan of care.
• Documents problems, goals, interventions and responses.
• Collaborates with the patient/family, attending physician and IDG.
• Trains and supervises patient/family in self-care when appropriate.
• Maintains accurate clinical notes on patient condition.
• Provides care in home settings regardless of patient demographics.
