The role requires an Ohio RN license, strong care coordination skills, and experience in acute or hospice care. Proficiency with EMR, Windows, and MS Office is expected to support documentation and communication.

Qualificações

• Current unencumbered RN license in Ohio required.
• Experience in acute care or medical-surgical nursing preferred.
• Hospice experience is a plus.

Responsabilidades

• Assess and plan patient care in home/hospice settings.
• Coordinate care with IDG and healthcare providers.
• Maintain accurate patient records and compliance with regulations.
