Ohio Living is seeking an experienced Home Health RN Case Manager to deliver high-quality in-home nursing care across the Greater Toledo area. You will assess patients, develop care plans, coordinate services, and collaborate with the care team to ensure timely interventions and patient comfort.

The role requires an Ohio RN license, at least one year of acute care experience, familiarity with EMR, and strong communication with patients and providers. Some evenings and weekends may be required.

Qualifikationen

• Current unencumbered Ohio RN license required.
• One year of acute care or medical-surgical nursing experience required.
• Experience in Medicare-certified home health agency preferred.
• Proficiency with Windows and EMR systems; strong communication skills.

• Care Management and Service Delivery - provide ongoing assessment and nursing care in patients' homes or facilities and develop a comprehensive plan of care.
• Quality and Compliance Management - maintain records and participate in quality improvement activities.

• Adhere to regulatory guidelines and agency protocols; ensure high quality customer service.
