Job summary

Affinity Health Management seeks a compassionate Registered Nurse to lead hospice care in home settings across communities. You will assess patient needs, develop and implement care plans with family involvement, and coordinate with the interdisciplinary team to ensure comfort and quality of life.

Responsibilities include comprehensive patient evaluations, medication administration as prescribed, education for patients and families, and on-call coverage.

Qualifications

• Graduate of an accredited nursing program.
• RN license in good standing to practice.

Responsibilities

• Complete initial and ongoing comprehensive assessments to determine hospice needs.
• Provide professional nursing care using the nursing process.
• Develop and revise plan of care with patient/family involvement.
• Administer medications and treatments as prescribed.
• Counsel patient and family on nursing and related needs.
• Maintain accurate clinical notes and communicate with physicians and care team.
• Participate in on-call duties and ensure necessary equipment and services are arranged.
• Support quality improvement initiatives within the hospice program.
