Job summary

Gentiva is seeking an experienced Hospice RN / RN Case Manager to lead patient care at home and support families facing life-limiting illness. You will coordinate with a multidisciplinary team to deliver individualized plans of care, perform assessments, and provide comfort and guidance in coordination with physicians and other professionals.

Applicants must hold an active PA RN license, have 1 year of hospice/home health experience, and maintain CPR certification.

Qualifications

Active RN license in Pennsylvania.
Minimum 1 year of RN experience in hospice, palliative care, or home health.
CPR certification required.
Hospice or palliative care experience preferred.

Responsibilities

Perform comprehensive patient assessments and reassessments, and provide education to patients, families and caregivers.
Deliver skilled nursing interventions and symptom management per plans of care.
Develop and update individualized care plans and visit frequency.
Coordinate care with interdisciplinary teams including physicians and nurses.
Educate patients and caregivers about disease, self-care, and end-of-life care.
Communicate changes in condition to the care team for timely interventions.
Document timely clinical records and maintain care plans.
Support continuity of care and transitions between levels of care.
Participate in on-call and patient support activities as applicable.
