Job summary

Gentiva is seeking an experienced Hospice RN / RN Case Manager to lead patient care in home, assisted living, or other patient residences. You will guide families, perform initial and ongoing assessments, and document care plans to ensure comfort and dignity for patients with life-limiting illnesses.

You will collaborate with physicians, LPNs, CNAs, social workers, and chaplains to tailor plans of care, educate families, and manage symptom relief in line with the hospice philosophy.

Qualifications

• Must be a registered nurse with an active license in the state of employment.
• Minimum of 1 year of RN experience in acute care, oncology, long-term care, hospice, or home health; hospice or palliative care preferred.

Responsibilities

• Perform comprehensive patient assessments and reassessments, providing education to patients, families, and caregivers.
• Deliver skilled nursing interventions and symptom management aligned with goals of care.
• Develop and update individualized plans of care and determine visit frequency to meet needs.
• Coordinate care with physicians and interdisciplinary teams.
• Educate patients and caregivers on disease, self-care, end-of-life care, medications, nutrition, and dietary needs.
• Document timely clinical records and communicate changes in condition to the care team.
• Support care continuity across home health, palliative, and hospice transitions.
• Participate in on-call and patient support activities as applicable.
