Job summary

Gentiva is seeking a Hospice RN / RN Case Manager to lead home-based end-of-life care. You will assess patients, deliver nursing care per Plan of Care, and coordinate with an interdisciplinary team in patients' homes or assisted living settings.

Required is an active RN license with at least 1 year in hospice, home health, or acute care. Hospice experience is preferred; CPR and driving are expected.

Qualifications

• Current RN license in the state of employment.
• Minimum of one year of RN experience in hospice, acute care, or home health.
• Hospice or palliative care experience preferred.

Responsibilities

• Perform comprehensive patient assessments and reassessments, educating patients, families and caregivers as appropriate.
• Deliver skilled nursing interventions and symptom management per care goals.
• Develop and update individualized plans of care including visit frequency and risk identifications.
• Coordinate care across interdisciplinary teams with physicians and other/providers.
• Educate patients and caregivers on disease processes, self-care, and end-of-life care.
• Document timely clinical records, orders, progress notes and care plans.
