Job summary

Gentiva is seeking a Hospice RN Case Manager to lead patient assessments and deliver high-quality nursing care in patients' homes or other settings. You will collaborate with an interdisciplinary team to implement and adjust individualized plans of care, educate families, and ensure compassionate end-of-life support.

The role requires an RN license, CPR certification, and at least one year of RN experience in hospice, home health, or related areas.

Qualifications

• Must hold a current RN license in the state of employment.
• CPR certification required.
• Minimum 1 year RN experience in acute care, hospice, home health, or related settings.

Responsibilities

• Perform comprehensive RN assessments and ongoing reassessments.
• Deliver skilled nursing interventions per plan of care, in home or facility settings.
• Develop and update individualized care plans with visit frequencies.
• Coordinate care with physicians, LPNs, CNAs, social workers and chaplains.
• Educate patients and caregivers on disease, self-care, and end-of-life care.
