Job summary

Gentiva is seeking a Hospice RN / RN Case Manager to lead comprehensive in-home nursing care for patients with life-limiting illnesses. You will assess, plan, and execute patient care, collaborating with physicians, CNAs, social workers and chaplains to refine individualized care plans.

You will educate families and guide them through end-of-life care with compassion and expertise. Ideal candidates hold an RN license, CPR certification, and have at least 1 year of RN experience; hospice or home

Qualifications

• Must be a licensed RN with CPR certification.
• Minimum 1 year RN experience, hospice or home health preferred.

Responsibilities

• Perform comprehensive patient assessments and reassessments in home or facility settings.
• Deliver skilled nursing interventions and symptom management per Plan of Care.
• Coordinate care with interdisciplinary team to update plans of care.
• Provide education and support to patients, families, and caregivers.
