Gentiva is seeking a Hospice RN / RN Case Manager to deliver personalized, compassionate care in patients' homes or suitable settings. You will assess, plan, implement, and coordinate comprehensive nursing services, ensuring comfort and dignity for patients with life-limiting illness.

You will collaborate with an interdisciplinary team, educate families, document care, and participate in on-call activities. A current RN license and CPR are required, with hospice experience preferred.

Qualificações

• A minimum of one (1) year experience as an RN in acute care hospital, oncology, long-term care, hospice or home health.
• Hospice or palliative care experience preferred.
• Current license to practice nursing in the state of requested employment.
• CPR Certification.
• Current automobile insurance and valid driver’s license.

Responsabilidades

• Be the primary point of contact for patients and families guiding them through every step of their hospice journey.
• Deliver skilled nursing interventions and symptom management.
• Develop and update individualized plans of care with patient visit frequency.
• Coordinate care across interdisciplinary teams and consult with physicians and other providers.
• Provide education and bereavement guidance to patients, families, and caregivers.
