At Home Hospice, part of UnitedHealth Group, seeks a Registered Nurse Case Manager to deliver and evaluate hospice care plans in patients' homes within a 50-mile radius. You will collaborate with physicians and care teams to promote physical, psychosocial, spiritual well-being for dying patients and families.

Responsibilities include initial nursing evaluations, plan-of-care initiation, and documentation of interventions.

Qualifications

• Current and unrestricted RN licensure in the state of practice.
• 1+ years of clinical experience.
• Current driver's license, valid vehicle insurance, and access to a dependable vehicle, or public transportation.

Responsabilités

• Makes the initial nursing evaluation in determining eligibility for hospice services during visit within 48 hours of referral.
• Identifies patient/family needs and reassesses at least every 15 days.
• Initiates and coordinates the plan of care.
• Documents problems, goals, and responses to hospice care.
• Collaborates with patient/family, attending physician, and IDG members for daily care.
