Job summary

UnitedHealth Group's Heart of Hospice, part of LHC Group, seeks a Registered Nurse Case Manager to oversee the Plan of Care for hospice patients, collaborating with IDG and implementing strategies to promote physical, psychosocial, emotional, and spiritual well-being.

This full-time position emphasizes timely assessments, care coordination, and patient-family education, with a salary range above the market and comprehensive benefits.

Qualifications

• Current and unrestricted RN licensure in the state of practice.
• Current driver's license, valid vehicle insurance, and access to a dependable vehicle or public transportation.

Responsibilities

• Makes the initial nursing evaluation in determining eligibility for hospice services during visit within 48 hours of referral.
• Identifies the patient/family's needs and reassesses as needed, at least every 15 days.
• Initiates and coordinates the plan of care.
• Documents problems, goals, interventions, and patient/family responses to hospice care.
• Collaborates with the patient/family, attending physician and IDG members to provide daily care.
