Qualifications

• Current and unrestricted RN licensure in the state of practice.
• Valid driver’s license, vehicle, or access to public transportation.
• CPR certification or ability to obtain within 90 days of hire.
• MS state specifics: 1+ years RN experience; LPN 3 years may substitute for RN requirement.

Responsibilities

• Perform initial nursing evaluation to determine hospice eligibility within 48 hours of referral.
• Identify patient and family needs and re-assess at least every 15 days.
• Initiate and coordinate the plan of care; document goals, interventions and responses.
• Collaborate with patient/family, physician and IDG for daily care.
• Instruct and supervise patient/family in self-care as appropriate.
• Maintain accurate clinical notes and adapt care to patient conditions.
