Job summary

Elite Hospice, part of the Optum/LHC Group family, seeks a dedicated Hospice Registered Nurse Case Manager to lead the Plan of Care for each patient. You will collaborate with professionals to implement treatment strategies, supporting physical, psychosocial, emotional, and spiritual well-being while addressing death and dying with compassion.

Qualifications

• Current and unrestricted RN licensure in the state of practice is required.
• Current driver’s license, valid vehicle insurance, and access to a dependable vehicle or reliable public transportation.

Responsibilities

• Make the initial nursing evaluation within 48 hours of referral to determine eligibility for hospice services.
• Identify physical, psychosocial, and environmental needs and re-assess at least every 15 days.
• Initiate and coordinate the plan of care with the IDG and care team.
• Document problems, goals, interventions, and patient/family responses to hospice care.
• Collaborate with patient/family, attending physician and IDG to provide daily care.
• Instruct and supervise patient/family in self-care techniques when appropriate.
• Maintain accurate clinical notes on patient condition.
• Function in diverse home environments and maintain professional care standards.
