Job summary

UnitedHealth Group is seeking a Registered Nurse Case Manager to oversee the Plan of Care for hospice patients within Heart N Home Hospice. You will collaborate with a multidisciplinary team to promote physical, psychosocial, emotional, and spiritual well-being for patients and families.

The role requires initiating care within 48 hours of referral, coordinating the plan of care, and documenting progress while ensuring high-quality, compassionate patient care across diverse home settings.

Qualifications

• Current and unrestricted RN licensure in the state of practice.
• Current driver's license, valid vehicle insurance, and access to a dependable vehicle, or public transportation.
• 1+ years of clinical experience.
• Current CPR certification or ability to complete within 90 days of hire.

Responsibilities

• Makes the initial nursing evaluation to determine eligibility for hospice services within 48 hours of referral.
• Identifies physical, psychosocial, and environmental needs and re-assesses as needed.
• Initiates and coordinates the plan of care.
• Documents problems, goals, interventions, and patient/family response to hospice care.
• Collaborates with patient/family, attending physician, and IDG members for daily care.
• Instructs and supervises patient/family in self-care techniques when appropriate.
• Maintains accurate clinical notes.
