Job summary

At Home Hospice, part of the UnitedHealth Group family, is seeking a Registered Nurse Case Manager to deliver the Plan of Care for hospice patients, supporting physical, psychosocial, emotional, and spiritual well-being. The role requires an RN license, reliable transportation, and the ability to work collaboratively with a multidisciplinary team.

You will be evaluated for productivity and quality of care and participate in ongoing professional development.

Qualifications

• Current and unrestricted RN licensure in the state of practice.
• Preferred CPR certification or ability to obtain within 90 days of hire.

Responsibilities

• Makes the initial nursing evaluation to determine hospice eligibility within 48 hours of referral.
• Identifies physical, psychosocial, and environmental needs and reassesses at least every 15 days.
• Initiates and coordinates the plan of care while documenting goals and patient response.
• Collaborates with the patient/family, physician and IDG members to provide daily care.
• Instructs and supervises patient/family in self-care when appropriate.
• Maintains accurate clinical notes and supports diverse patients regardless of background.
