Job summary

UnitedHealth Group is seeking a Registered Nurse Case Manager to oversee the plan of care for hospice patients and to collaborate with clinicians to promote physical, psychosocial, emotional, and spiritual well-being.

Primary responsibilities include timely evaluation, care plan initiation, documentation, and coordinating with the IDG to support patients and families facing end‑of‑life decisions with compassionate, patient‑centered care.

Qualifications

• Current and unrestricted RN licensure in the state of practice.
• Current driver's license with access to reliable transportation or use of public transportation.
• Current CPR certification or ability to complete within 90 days of hire.

Responsibilities

• Make the initial nursing evaluation within 48 hours of referral.
• Identify the patient's needs and re-assess no less than every 15 days.
• Initiate and coordinate the plan of care.
• Document problems, goals, interventions, and patient/family response to hospice care.
• Collaborate with the patient/family, attending physician and other IDG members.
• Instruct and supervise the patient/family in self-care techniques when appropriate.
• Maintain accurate and relevant clinical notes regarding the patient's condition.
• Provide care in home settings without regard to age, race, creed, color, sex, disability, or financial condition.
