Job summary

UnitedHealth Group is seeking a Registered Nurse Case Manager to deliver in-home hospice care and manage the Plan of Care for each patient. You will collaborate with other professionals to promote physical, psychosocial, emotional, and spiritual well-being while navigating the needs of patients and families facing end-of-life care.

The role requires an active Ohio RN license, a valid driver's license, and willingness to travel within a 50-mile radius.

Qualifications

• Current and unrestricted RN licensure in the state of practice.
• Current driver's license, valid vehicle insurance, and access to a dependable vehicle, or public transportation.
• 1+ years of clinical experience (preferred).
• Current CPR certification or ability to complete within 90 days of hire.

Responsibilities

• Makes the initial nursing evaluation for hospice eligibility within 48 hours of referral.
• Identifies physical, psychosocial, and environmental needs and reassesses every 15 days.
• Initiates and coordinates the plan of care with the care team.
• Documents problems, goals, interventions, and patient/family response to hospice care.
• Collaborates with patient/family and IDG for daily care.
• Instructs and supervises the patient/family in self-care techniques when appropriate.
• Maintains accurate clinical notes.
