Job summary

UnitedHealth Group is seeking an RN Case Manager for in-home hospice services. You will perform initial evaluations, develop the patient’s plan of care, and coordinate with the care team to ensure high-quality, compassionate care within a 50-mile service radius.

Texas RN licensure and 1+ year hospice experience are required, with a valid driver's license and reliable transportation. Salary is based on full-time employment with a comprehensive benefits package and potential incentives.

Qualifications

• Active Texas RN license required.
• Minimum 1 year of clinical hospice or home health experience preferred.
• Valid driver’s license and reliable transportation required.

Responsibilities

• Conduct initial nursing evaluation for hospice eligibility within 48 hours of referral.
• Develop and adjust the Plan of Care in collaboration with the IDG.
• Document patient/family needs, goals and responses to care.
• Coordinate care with physicians and other team members.
• Provide patient/family education and self-care guidance.
