Job summary

Care Initiatives Hospice is seeking a compassionate Registered Nurse (RN) Case Manager to support patients and families in various settings, including home, long-term care, assisted living, or hospital. You will lead high-quality care, conduct assessments, and collaborate with an interdisciplinary team to honor patient needs with dignity and respect.

The role requires an Iowa RN license, CPR certification, hospice experience, and the ability to travel within the region.

Qualifications

Active Iowa RN license.
Active CPR certification.
Hospice or palliative care experience.
Two years of verifiable RN/LPN experience preferred.
Current driver's license, auto liability insurance, and ability to travel between multiple locations within your region.
Home access to the internet.
Ability to obtain Hospice and Palliative RN Certification (CHPN) within 24 months of employment.

Responsibilities

Deliver high quality patient care in the patient’s place of residence.
Develop and oversee plan of care under the direction of the physician and ensure interdisciplinary involvement.
Perform initial and comprehensive assessments on all patients.
Continual visits and interdisciplinary assessment of the patient.
Deliver clear, concise clinical documentation.
Provide patient, caregiver and family counseling and education as indicated in the plan of care.
Develop and continually update plan of care to meet patient-specific needs.
