Qualifications

• Current license to practice nursing in the state of employment.
• CPR Certification.
• Current automobile insurance and valid driver's license.
• One year RN experience; hospice or palliative care preferred.

Responsibilities

• Be the primary contact for patients and families, guiding them through the hospice journey.
• Deliver and document skilled nursing care per Plan of Care in home or community settings.
• Perform initial and ongoing assessments and adjust care as needed.
• Collaborate with physicians, LPNs, CNAs, social workers, chaplains, and others.
• Provide emotional, physical, and spiritual support to patients and families.
• Educate caregivers and ensure safe, confident home care.
• Maintain accurate clinical documentation and ensure infection control compliance.
• Support quality improvement and scheduling initiatives.
