Job summary

Gentiva is seeking a compassionate Hospice RN / RN Case Manager to lead patients and families through the hospice journey in home settings. You will deliver skilled nursing care per an individualized Plan of Care and collaborate with an interdisciplinary team to refine care plans.

The role emphasizes education, bereavement support, and safe, compassionate end-of-life care, with emphasis on documentation and regulatory compliance. A valid RN license and CPR are required.

Qualifications

• Active RN license in the state of employment.
• Minimum 1 year experience as an RN; hospice/home health preferred.
• CPR certification required.
• Reliable transportation and valid driver’s license.

Responsibilities

• Serve as primary contact for patients and families throughout the hospice journey.
• Deliver and document skilled nursing care per Plan of Care in home/ facility.
• Perform initial and ongoing patient assessments.
• Collaborate with IDG to refine care plans.
• Provide education and bereavement support to families.
• Teach caregivers and ensure safe home care.
• Maintain accurate clinical documentation and infection control.
• Support quality improvement and scheduling initiatives.
• Communicate patient condition updates to the care team.
• Coordinate across interdisciplinary teams.
