Job summary

Gentiva is seeking a compassionate Hospice RN / RN Case Manager to lead patients and families through the hospice journey in home or community settings. You will perform assessments, deliver hands-on nursing care, and document care plans with a focus on comfort, dignity, and family education.

You will collaborate with an interdisciplinary team to tailor plans of care, educate caregivers, and ensure timely communication of any clinical changes, while maintaining high standards of documentation

Qualifications

• Active RN license in Indiana.
• Hospice or palliative care experience preferred.
• Strong communication and compassionate patient care.

Responsibilities

• Perform comprehensive patient assessments and reassessments in home or related settings.
• Deliver skilled nursing interventions and symptom management per plan of care.
• Develop and update individualized plans of care and visit frequency.
• Coordinate care across interdisciplinary teams including physicians and social workers.
• Educate patients and caregivers on disease, end-of-life care, medications, nutrition, and self-care.
• Communicate changes in condition to the care team and document promptly.
• Maintain accurate clinical records and ensure infection control standards.
• Support scheduling, quality improvement, and care continuity initiatives.
