Job summary

Gentiva is hiring a Hospice RN / RN Case Manager to deliver personalized in-home nursing care for patients facing life-limiting illness. You will conduct assessments, implement plans of care, and coordinate with an interdisciplinary team to ensure compassionate, holistic support.

Responsibilities include education for families, symptom and pain management, and thorough documentation. This role emphasizes dignity, comfort, and bereavement guidance in patients’ homes and communities.

Qualifications

• Experience in home hospice or palliative care is preferred.
• Ability to perform comprehensive nursing assessments and care planning.
• Strong communication with patients, families and IDG.

Responsibilities

• Perform comprehensive patient assessments and reassessments in home settings.
• Deliver nursing interventions and symptom management per care plans.
• Coordinate care with physicians, social workers, and other team members.
• Educate patients and caregivers on disease, medications, and self-care.
• Document care accurately and maintain compliance with regulations.
