Job summary
Qualifications
- Active RN license in state of employment.
- Hospice or palliative care experience preferred.
- Strong communication with patients, families, and care teams.
Responsibilities
- Perform comprehensive patient assessments and reassessments in homes or settings.
- Deliver nursing interventions and symptom management per Plan of Care.
- Develop/update plans of care and visit frequency.
- Coordinate with physicians, LPNs, CNAs, social workers and chaplains.
