Job summary

Gentiva is seeking a Hospice RN / RN Case Manager to lead compassionate, in-home care for patients facing life-limiting illness. You will assess, plan, and deliver skilled nursing care, coordinating with physicians, CNAs, social workers, and chaplains to ensure comprehensive, patient-centered plans of care.

You will educate families, document care accurately, and participate in on-call support while maintaining regulatory compliance and infection control standards.

Qualifications

• Current license to practice nursing in the state of requested employment
• CPR Certification
• Minimum of 1 year experience as an RN in acute care hospital, oncology, long-term care, hospice or home health; hospice or palliative care experience preferred

Responsibilities

• Perform comprehensive patient assessments and reassessments, including in-person or telephonic RN assessments and education for patients, family members and caregivers.
• Deliver skilled nursing interventions and symptom management aligned with goals of care.
• Develop and update individualized plans of care including visit frequency to meet needs and risk.
• Coordinate care across interdisciplinary teams including physicians and nurse practitioners.
• Provide patient and caregiver education on disease process, end-of-life care, medications, nutrition, and dietary needs.
• Communicate changes in condition to the care team for timely interventions.
• Support continuity of care by coordinating home health, palliative and hospice referrals and transitions in levels of care.
• Document timely clinical records including orders, progress notes, and care plans.
• Maintain effective relationships with IDG and involve team in plan of care as needed.
• Participate in on-call and patient support activities as applicable
