Job summary

Gentiva Hospice is seeking an experienced Hospice RN / RN Case Manager to deliver personalized, compassionate end-of-life care in patients' homes or other settings. You will perform assessments, implement plans of care, and coordinate with an interdisciplinary team to support patients and families.

Responsibilities include education for caregivers, symptom management, timely documentation, and ensuring regulatory compliance.

Qualifications

• Active RN license in the state of employment.
• Hospice or palliative care experience preferred.
• Strong assessment and communication skills.

Responsibilities

• Perform comprehensive patient assessments and ongoing reassessments.
• Deliver skilled nursing interventions and symptom management.
• Develop and update individualized plans of care and visit schedules.
• Coordinate care with interdisciplinary team.
• Educate patients and caregivers on disease, care, and end-of-life options.
• Document clinical findings and care plans accurately.
• Maintain compliance with regulatory standards.
