Job summary

New Day Hospice seeks a Registered Nurse Case Manager to administer nursing care in patients' homes and coordinate with the interdisciplinary team, families, and referring agencies. Qualifications

• Graduate from an accredited Diploma, Associate, or Baccalaureate School of Nursing.
• Current RN licensure in state and/or Nurse Licensure Compact rules.
• Two years RN experience in a clinical care setting; hospice experience preferred.
• Nursing skills per standards of practice; good interpersonal skills.
• Must read, write and comprehend English; valid driver's license and reliable transportation.

Responsibilities

• Initial and ongoing comprehensive assessments of patients' needs and eligibility for hospice care.
• Develop and document the plan of care to ensure quality and continuity of care.
• Educate patients and families about disease process, self-care, end-of-life care, nutrition, and dietary needs.
• Determine scope and frequency of services based on acuity and family needs.
• Provide on-call coverage and coordinate with the interdisciplinary team for patient care.
• Coordinate care with hospice team, Aides, and physicians; ensure documentation and communication.
• Promote hospice philosophy and administrative policies; participate in in-service programs.
