Job summary

New Age Hospice seeks a Registered Nurse Case Manager to provide in-home nursing care and coordinate treatment with the Interdisciplinary Group and families. You will assume responsibility for planning and coordinating patient care across the care team.

Qualifications include an RN license, 2 years of nursing experience (hospice preferred), strong communication skills, and the ability to work in varied home environments with flexibility and travel as needed.

Qualifications

• RN licensure (state) and license to practice as an RN.
• Minimum 2 years RN experience in clinical care; hospice experience preferred.
• Excellent clinical assessment and care planning abilities.
• Strong reading/writing English communications with patients and families.

Responsibilities

• Perform comprehensive nursing assessments in patients' homes.
• Coordinate care with Interdisciplinary Group/Team and families.
• Develop and implement plans of care with ongoing evaluation.
• Provide on-call support and point of contact for patient needs.
