Job summary

Requirements include an RN license, 2+ years of RN experience, and strong communication skills in English; a driver's license is required for local travel.

• Current state RN license required.

Responsibilities

• Perform initial and ongoing assessments of patients in home settings.
• Coordinate care with IDG/Team, families, and referring agencies.
• Develop and document plans of care and ensure continuity of services.
• Educate patients and families on disease process, self-care, and end-of-life care.
• Determine scope/frequency of services based on acuity and needs.
• Provide on-call coverage and respond to urgent needs.
