Job summary

New Day Hospice seeks a Registered Nurse Case Manager to administer nursing care in patients' homes and coordinate with the interdisciplinary team, families, and referring agencies. The role emphasizes high-quality end-of-life care, documentation, and collaborative care planning.

Responsibilities include conducting assessments, developing and updating the plan of care, and providing patient and family education. Requires RN license, 2+ years in clinical care, and reliable transportation.

Qualifications

• Graduate from an accredited Nursing program (Diploma/Associate/Bachelor).
• Current RN license in the state; valid driver's license may be required.
• Two years RN experience in clinical care; hospice experience preferred.

Responsibilities

• Perform nursing care for patients of all ages in home settings.
• Coordinate care with IDG/Team, families, and referring agencies.
• Document progress and communicate changes to physicians and care team.
• Provide education and support to patients and families about disease and care plans.
• Participate in QAPI and on-call responsibilities as assigned.
