Job summary

Horizons Hospice is seeking a compassionate Registered Nurse to administer nursing care for patients at their residence, coordinating with the interdisciplinary team, patient, family, and referring agency. The role emphasizes comprehensive, patient-centered hospice care and collaboration with the IDG.

Two years of RN experience in a clinical setting is preferred; hospice experience is a plus. Strong communication and English literacy are essential for documentation and family education.

Qualifications

• Specialized palliation and end-of-life care experience preferred.

Responsibilities

• Initial and ongoing assessments of physical, functional, psychosocial, and environmental needs.
• Determine admission criteria and eligibility for hospice care.
• Develop and document the plan of care to ensure quality and continuity of service.
• Educate patient and family about disease, self-care, end-of-life care, nutrition; train staff as needed.
• Assess caregiver ability to meet needs and coordinate services accordingly.
• Provide clinical directions to aides and LPN/LVN to ensure quality care.
• Prepare progress notes documenting milestones toward goals.
• Inform physician of changes and evaluate patient/family response to care.
• Monitor cases for third party payor requirements.
• Participate in on-call responsibilities and communicate with the team.
