Job summary

New Day Hospice seeks a Registered Nurse Case Manager to administer nursing care and coordinate patient care in home settings. You’ll work with the Interdisciplinary Group to ensure smooth transitions and quality outcomes.

Ideal candidates have 2+ years RN experience, hospice exposure preferred, strong communication skills, and current RN licensure with reliable transportation. Flexibility for travel and on-call coverage is required.

Qualifications

• Graduate from an accredited nursing program (Diploma/Associate/Baccalaureate).
• Current RN license in state; valid driver's license.
• Minimum 2 years RN experience in clinical setting; hospice preferred.
• Excellent interpersonal and communication skills; English proficiency required.
• Must read, write and understand English; educate patients/families.
• Reliable transportation for home visits.

Responsibilities

• Initial and ongoing nursing assessments in patients' homes.
• Coordinate care with the IDG/Team and families.
• Develop and revise plan of care with physician input.
• Provide end-of-life care and palliation education.
• Document progress notes and ensure continuity of care.
• On-call responsibilities as assigned.
• Monitor care coordination with third-party payors.
