Job summary

Advocate Health Care seeks an experienced RN to coordinate care for terminally ill patients and families within hospice settings. The role emphasizes timely case management, teaching, and documentation to ensure high-quality end-of-life care.

The RN will lead the IDG, update care plans, and communicate with physicians and team members. A nursing license and relevant hospice experience are required.

Qualifications

• RN license in the state of practice.
• Completion of an accredited nursing program.
• 1–2 years of nursing experience in home health, hospice, medical/surgical, LTC, ER/ICU or oncology is typical.

Responsibilities

• Coordinate patient and family care in hospice settings with planned contacts.
• Lead the IDG to develop and implement the Plan of Care for each patient.
• Document and update the Electronic Health Record per standards.
• Educate patients and families about disease trajectory and end-of-life care.
• Facilitate discharge planning and coordination of services after discharge.
