Job summary

New Day Hospice seeks a Registered Nurse Case Manager to administer nursing care for patients in their homes and coordinate care with the Interdisciplinary Group/Team, families and referring agency. The RN will assume responsibility for planning and monitoring patient care throughout the care continuum.

The ideal candidate holds an RN license, at least two years of clinical experience, and is proficient in English.

Qualifications

• Graduate from an accredited Diploma, Associate, or Baccalaureate School of Nursing.
• Current RN license and drivers license.
• Two years RN experience in a clinical care setting; hospice experience preferred.
• Must read, write and comprehend English.

Responsibilities

• Perform initial and ongoing nursing assessments for patients in their homes.
• Coordinate care with the Interdisciplinary Group/Team, families and referring agency.
• Develop, implement and update the plan of care to ensure quality and continuity of service.
• Provide education to patients, families and caregivers regarding disease process and end-of-life care.
• Communicate changes in patient needs to physicians and team members.
• Assist in on-call coverage and respond to emergencies as assigned.
• Ensure accurate documentation and adherence to third-party payor requirements.
