Job summary

New Day Hospice is seeking a Registered Nurse Case Manager to administer nursing care for patients in their homes and coordinate care with the interdisciplinary team and families. The role involves travel to patient homes, on-call responsibilities, and ensuring quality, continuous care aligned with hospice guidelines.

Minimum two years of RN experience in clinical settings is required, with hospice experience preferred.

Qualifications

• Graduate from an accredited Diploma, Associate, or Baccalaureate School of Nursing.
• Current state license as an RN and/or Nurse Licensure Compact rules; valid driver’s license.
• Two years RN experience in a clinical setting; hospice experience preferred.
• Nursing skills per standards; good interpersonal skills; read/write English.
• Reliable transportation and valid auto liability insurance.

Responsibilities

• Initial and ongoing comprehensive assessments of patient needs (physical, functional, psychosocial, environmental).
• Develops and documents plan of care to ensure quality and continuity of care; recommend revisions as needed.
• Educate patients and families on disease process, self-care, end-of-life care, nutrition, and dietary needs; train staff as needed.
• Coordinate care with the Interdisciplinary Group/Team; monitor acuity and family needs; ensure proper documentation.
• Provide on-call services to patients and families as assigned; communicate changes to physicians.
