Job summary

Lifecare Home Health and Hospice seeks an experienced RN Case Manager to perform admissions and administer nursing care for patients in their homes. You will coordinate with the Interdisciplinary Team, patients, families, and referring agencies to ensure seamless care across settings.

Responsibilities include assessing patients, developing care plans, on-call duties, and ensuring timely documentation while adhering to hospice quality and regulatory standards.

Qualifications

• Nursing skills as defined by generally accepted standards of practice.
• Knowledge of state and federal regulations governing nursing care.
• Excellent interpersonal and communication skills with patients, families, and team members.

Responsibilities

• Assess hospice patients using Nursing Assessment and LCD criteria to identify physical, psycho-social, and environmental needs.
• Develop and document the plan of care in correlation with the assessment.
• Provide nursing care with infection control measures (OSHA).
• Collaborate with the Interdisciplinary Group/Team to meet patient/family needs.
