Job summary

New Age Hospice is seeking a dedicated Registered Nurse Case Manager to deliver high-quality in-home nursing care across ages. You will coordinate care with the Interdisciplinary Group, families, and referring agencies, ensuring individualized plans and timely interventions.

The role requires clinical excellence, strong communication, and the ability to travel locally to patients' homes while maintaining compassionate, patient-centered support.

Qualifications

• Education: Graduate from an accredited Diploma, Associate, or Baccalaureate School of Nursing.
• Licensure: Current RN license and driver’s license.
• Experience: Two years as an RN in a clinical care setting, hospice preferred.
• Skills: Nursing skills, good interpersonal skills, and end-of-life care familiarity.
• Transportation: Reliable transportation and valid auto liability insurance.

Responsibilities

• Initial and ongoing comprehensive assessments of patient needs in the home.
• Develop and implement plans of care to ensure quality and continuity.
• Coordinate with the IDG/Team, families, and referring agencies.
• Educate patients and families on disease process, self-care, and end-of-life care.
• Provide clinical directions to aides and LPNs/LVNs and maintain documentation.
