Job summary

Affinity Hospice Management LLC seeks a dedicated Hospice Aide in Connecticut. The role supports licensed nursing staff, delivering essential personal care to patients and promoting comfort in line with each patient’s plan of care.

Responsibilities include bathing, grooming, safety, documentation in EMR, and timely reporting of changes to the RN Case Manager. This is a full-time, on-site position in the Fairfield area.

Qualifications

• Completion of a state approved certified nursing assistant course or equivalent training.
• Nursing Assistant Certified (NAC).
• Current Healthcare Provider CPR certification.
• Current Food Handler Permit.
• High school diploma or GED.
• Ability to read and follow written instructions and document care given.
• Understands hospice philosophy and caring for terminally ill patients.
• Prior experience in hospice, long-term care or similar settings preferred.
• Basic computer skills to document in EMR.
• At least 18 years of age.
• Licensed driver with insured vehicle in good working order.

Responsibilities

• Perform personal care tasks as assigned, promptly addressing patient requests.
• Provide positive communication to patient and family while maintaining confidentiality.
• Ensure safety and proper use of equipment during care activities.
• Assist with baths, hygiene, bed linen changes, and grooming.
• Assist in dressing and undressing patients as assigned.
• Assist with feeding; avoid homemade foods from patient or caregiver.
• Observe and report skin condition to prevent tissue breakdown.
• Report pain or changes to RN Case Manager; monitor caregiver dynamics.
• Assist in ambulation and prescribed exercise by hospice staff.
• Provide ROM and transfers as directed.
• Assist with self-administered medications as allowed by state regulations.
• Offer bedpans/urinals and perform light housekeeping tasks.
• Perform ROM and basic procedures as competency is demonstrated.
• Provide respite for patients/families when on-site, as appropriate.
• Keep living area clean and orderly; follow care procedures and standards.
• Participate in quality improvement activities as required.
• Respect patient/family environment and personal needs.
• Act as mandatory reporter for health department requirements.
• Report suspected abuse or neglect immediately as required.
