Job summary

Qualifications

• Completion of a state approved certified nursing assistant course or equivalent training.
• Nursing Assistant Certified (NAC).
• Current Healthcare Provider CPR certification (hands-on).
• Current Food Handlers Permit.
• High school diploma or GED.
• Ability to read and follow written instructions and document care given.
• Understands hospice philosophy and comfortable providing specialized care to the terminally ill.
• Prior experience in hospice, orderly, acute, rehabilitative, or long term care facility. 1 year preferred.
• Basic computer skills to document in EMR.
• At least 18 years of age.
• Licensed driver with automobile insured and in good working order.

Responsibilities

• Performing personal care after assignment and attending to patient requests promptly.
• Provide patient and family with positive communication techniques, including confidentiality.
• Meet safety needs of patient and use equipment safely.
• Give personal care including baths, back rubs, oral hygiene, shampoos and changing bed linen.
• Assist in dressing and undressing patients as assigned.
• Assist in feeding the patient as assigned.
• Provide care and observation of patient's skin to prevent breakdown.
• Report pain or changes to RN Case Manager.
• Assist in ambulation and exercise as instructed by the hospice nurse or therapist.
• Provide ROM and transfers/positioning as directed.
• Assist with self-administered medications as allowed by state regulations.
• Offer and assist with bedpans and urinals; light housekeeping tasks.
• Perform ROM and simple procedures with demonstrated competency.
• Provide respite for patient/family/caregiver on-site as appropriate.
• Keep patient's living area clean and orderly.
• Adhere to documentation and care procedures and standards of conduct.
• Participate in quality assessment and improvement activities.
• Respect patient and family environment and personal needs.
• As a mandatory reporter, report non-compliance to Dept. of Health within 14 days.
• Report abuse, neglect, or exploitation to proper authorities immediately.
