Job summary

Qualifications

• Completion of a state-approved CNA program or equivalent.
• NAC certification.
• CPR certification (hands-on preferred).
• High school diploma or GED; valid driver’s license.

Responsibilities

• Perform personal care tasks and respond promptly to patient requests.
• Communicate with patients and families in a respectful, confidential manner.
• Ensure safety and proper use of equipment during care.
• Assist with bathing, oral hygiene, grooming and linen changes.
• Assist with dressing, feeding, and mobility as directed.
• Observe and report pain or changes in condition to the RN Case Manager.
• Support self-care and comfort through range of motion and transfers.
• Assist with medications reminders as allowed by state rules.
• Maintain clean living area and complete required documentation.
